Don’t feel that you have to be too elaborate with these answers. Short answers are fine.
We just need some information about the personality of your practice and then we can take it from there.

2. Who will be the point of contact, make decisions about the design and facilitate the progression of the design process? (Name, Title and Contact Information)
3. What do you want to achieve with the new design?
4. Describe your practice in 1-2 sentences.
5. Describe your ideal patient (income, demographics and type of procedures)
6. Describe your typical patient (income, demographics and type of procedures)
7. What image about you and your practice do you want to portray to potential patients? Give details about your brand.
8. Is there a specific look and feel that you have in mind? Describe your ideal website style.
9. List some style aspects that you do NOT want, if any, that you think do not match your target audience of brand. What do you NOT want your website to look like?
10. Do you want to keep your current website colors? If not, what colors would you be interested in using?
11. Are you interested in a new logo? This is a separate project.

16. Are you able to provide us with high quality images to be used on your website? Do you have permission to use these images? (office photos, doctor/staff photos, logo, patient photos) Note - when you send these be sure they are full size - not cropped or downsized.
17. Do you use social media? If so please list the social media sites you currently use and provide their URL or username so we can place them on your new site.
18. List aspects about your career that help you stand out from some of your competition or information that you'd want potential patients to know (AACD, lecturer, awards won, etc.)
19. Additional comments you think would be helpful to know when creating your new website.
